Grady Memorial Hospital, Five Oaks Medical Group, Rush Springs Family Medical Clinic, and Tuttle Family Medical Clinic are pleased to announce our new Patient Portal. This interactive web portal will empower patients and their families to take a more active role in their care by providing easy, secure access to their health information and online communication with Clinic staff.
Patient Portal gives patients instant access to their health information, including laboratory and radiology results, medications, allergies, demographic information, visit history, discharge/aftercare information, and more. In addition to handling their own care, parents and health care proxies with authorized consent will appreciate the ability to access the records of those individuals for whom they manage care.
Patient Portal allows patients to view the central appointment page which displays their list of scheduled and pending appointments. Here, they can review appointment details.
Patient Portal also saves patients time by allowing them to submit updates to their demographic and contact information prior to their visits.
Patients can use Patient Portal to communicate with their physicians, as well. They can request prescription renewals online instead of calling the office. Email notifications alert them to any new activity on their portal such as test results, messages, or appointments.
This website is best viewed in Google Chrome, Microsoft Edge and Firefox. It will not work with older versions. Users assume sole responsibility for configuration, security, or content of their particular device. Grady Memorial Hospital assumes no liability in this regard.

Frequently Asked Questions
Pediatric Proxy Access
In order to get access to your child's My Health Info Patient Portal, you must fill out the Proxy Request Form and bring it to the Pediatric Department, your Family Medicine provider's office, or the Medical Records Department at Grady Memorial Hospital. This form may also be filled out at the appointment. If you are not a parent but you have legal guardianship, you must bring a copy of your legal guardianship papers in order to get access to a child's portal.
Proxy Request Form
If you have your own personal My Health Info Portal, your child will be added to it. If you do not have a personal portal, one will be created for your child and the log in information will be sent to your email. Access to your child's portal will automatically terminate when the child turn's 18. If your child is 18 or over and would like for their parent(s) to have access to their portal, they may fill out the Proxy Access Form to give their parent(s) access.
Adult Proxy Access
In order to have Proxy Access to another adult's portal, the Proxy Request Form must be filled out. If you are the Power of Attorney for Healthcare decisions or Healthcare Proxy, the Proxy Access Form must be filled out and legal documentation presented in order to gain portal access for that person. If the patient is coherent, they must also sign the Proxy Access Form. Proxy access may be revoked by the patient at any time by filling out the Proxy Revocation Form.
